Some of the results achieved to date include: development and validation of regional policies on sample transport; ECOWAS regional disease outbreak preparedness and response plan; establishment of steering committees and technical working groups in all countries; establishment of the One Health Regional Committees[1] across West Africa; delivery of regional technical assistance to countries by WAHO, the World Health Organization (WHO), World Animal Health Organization (OIE), Centre for International Cooperation in Health and Development (CCISD), Fondation Merieux, and the Regional Animal Health Centre (RAHC) in Bamako; development of advanced level (Master’s) Field Epidemiology and Laboratory Training Program (FELTP) with the University of Ghana and University of Ouagadougou for a first cohort of 50 participants; dissemination of epidemiological bulletins; implementation of regional disease outbreak simulation exercise on Yellow Fever; and strengthening[2] of 47 health district Centers of Epidemiological Surveillance (CES) in Guinea, Guinea-Bissau, Liberia, Sierra-Leone and Togo. The overall disbursement rate of REDISSE 1 is 22%. Disbursement of financing for REDISSE 1 countries has been improved despite some delays in submission and implementation of Annual Work Plans & Budgets (AWPB). It is expected to accelerate from mid-2019 as many procurement actions are pending delivery and payment. Once this acceleration has been documented, the task team will consider upgrading project performance to satisfactory. Some activities under the 2018 AWPB could not be completed in 2018 and have been carried into the 2019 AWPB for the three REDISSE 1 countries. The most important difficulties encountered are: (i) procurement delays (preparation of procurement plans, calls for expressions of interest, TORs etc. and inadequate familiarity/comfort with the STEP system, (ii) delays in recruiting permanent project staff and high staff turnover due to changes in government; (iii) management of the national "One Health" platform and difficulties working across sectors in the planning and implementation processes; and(iv) infrequent updating of information for the results framework. Guinea: REDISSE 1 in Guinea has been under implementation for two years. The disbursement rate is 14% and is expected to increase to 30% by June 2019. Rehabilitation of the Project Implementation Unit building and rehabilitation of a modern laboratory in the Ministry of Livestock have been planned under the 2018 AWPB and should start before the end of June 2019. Senegal: The disbursement rate has been largely improved to 27% despite delays in execution of 2018 AWPB. 40% only of the planned activities have been executed in 2018 and the remaining activities have been carried out into the 2019 AWPB. Important delays are noted on procurement processing with some important contracts remaining to be finalized. The necessary diligence must be provided for a proper execution of all ongoing contracts and their timely payment to ensure a substantial improvement of disbursement. Sierra Leone: Out of 5 country-level PDO indicators, Sierra Leone has met the annual target on 4 with only one exception on “Availability of human resources to implement IHR core capacity requirements”. The National Steering Committee (NSC) for REDISSE has been established (10th May 2018), the PIM was revised in May 2018, and the Contingency Emergency Response Component (CERC) Operations Manual (OM) was developed by two ministries and approved the World Bank in June 2018. REDISSE disbursement for Sierra Leone is currently at 19.4%. By May 1, 2019, The Integrated Health Project Administration Unit (IHPAU) restructuring was completed and all the staff are on board. To improve the implementation of REDISSE for project management, the country team had recommended a workshop to familiarize all the relevant directorates and PIU staff with the REDISSE Financial Agreement, PIM, and Results Framework during the last mission. Highlights of progress under the Project include: (a) further improvement in promotion and expansion of e-IDSR and launch of even based community infectious diseases by the animal health sector; (b) the project supported the successful measles outbreak response in one district; and (c) two candidates were sent to Ghana for the advanced field epidemiological training. A new AWPB tool developed enabling the REDISSE countries to track activities being implemented under the project has been shared with the clients. However, REDISSE in Sierra Leone has experienced delays in implementation. By the time of last mission in December 2018, only 47% (56/119) of planned activities under the 2018 AWPB were completed or under implementation. There has been no progress under the Component 2. Besides, there has been little progress in implementing “big ticket” items such as risk-based surveillance, regional centers, etc. It also suffers from poor quality and timely submission of reports. The submission of AWPB 2019 to the WBG was delayed. Other challenges encountered in the implementation of REDISSE in Sierra Leone include: (i) poor quality of Terms-of References (ToRs); (ii) delays and poor quality of procurement works; (iii) weak internal tracking of implemented activities; (iv) AWP&B: lack of alignment of AWPB with the National Action Plan for Public Health Security (NAPHS) in 2018 and weak collaboration between animal and human health sectors for joint planning; and (v) lack of collaboration among other donor supported activities and projects. ECOWAS/West African Health Organization (WAHO): Implementation of Program activities and disbursement of regional funds managed by WAHO has been swift and is accelerating. To-date, 34% of the IDA grant and 68% of the MDTF grant have been disbursed. The WAHO disbursement rate is higher than originally projected, and further accelerating due to the REDISSE Program expansion. Full implementation of the WAHO 2019 Annual Work Plan and Budget (AWPB) will result in a projected 83 percent disbursement rate by December 31, 2019. The REDISSE Program could run out of funds to support most regional activities as early as June 30, 2020, while the project end date is January 31, 2023. According to budget projections an additional US$ 29 million is needed to support the regional activities until the end of the REDISSE Phase 1 project, bringing the total for regional activities to US$ 49 million, or 11 percent of the total IDA investment in REDISSE West Africa. An additional financing of USD10.5 from the MDTF Canada is under preparation and the team plans to work with WAHO to address the projected funding shortfall through resource mobilization from IDA 19 and other sources.
